These Terms and Conditions govern your use of the Let It Be Wellness website and services offered by Let It Be Wellness, located in Sun City, Arizona (“Let It Be Wellness,” “we,” “us,” or “our”). By accessing this website, scheduling services, purchasing products, or enrolling in a membership, you agree to these Terms.
 

1. Website Information and Medical Disclaimer

Information provided on this website is for general informational and educational purposes and is not intended to replace individualized medical advice, diagnosis, or treatment.

Accessing the website, submitting an inquiry, or purchasing a nonclinical product does not by itself establish a healthcare provider-patient relationship. Clinical treatment is provided only after an appropriate evaluation and acceptance by a qualified healthcare professional.

Eligibility for IV therapy, injections, medications, ultrasound or imaging services, supplements, or other treatments is determined by the treating provider based on medical history, examination, clinical appropriateness, and applicable law.

If you believe you are experiencing a medical emergency, call 911 or seek emergency medical care immediately.
 

2. No Guarantee of Results

Individual responses to wellness services, medications, supplements, IV therapy, and other treatments vary. Let It Be Wellness does not guarantee any specific medical, cosmetic, wellness, or other outcome.

Any testimonials, photographs, descriptions, or examples displayed on the website represent individual experiences and should not be interpreted as a promise of identical results.

Health-related advertising must be truthful, non-misleading, and supported by appropriate evidence under federal standards.
 

3. Appointments and Services

Appointments are subject to availability and provider approval.

We may decline, postpone, modify, or discontinue a requested treatment when a provider determines that the service is medically inappropriate or unsafe.

Patients are responsible for providing accurate and complete health, medication, allergy, pregnancy, and other relevant information before treatment.

Separate informed-consent documents may be required for individual medical procedures and treatments.
 

4. Pricing and Payment

Prices displayed on the website are subject to change prospectively unless otherwise stated.

Payment may be required before or at the time services or products are provided.

Patients are responsible for charges for services they authorize except where another written payment arrangement applies.

Promotional offers and discounts are subject to their stated terms and generally may not be combined unless expressly authorized.
 

5. Wellness Membership

Let It Be Wellness offers an optional $30-per-month Wellness Membership.

Membership automatically renews monthly until canceled. Members receive applicable member discounts and up to $30 in membership credit toward eligible Let It Be Wellness services during the applicable membership period, subject to the separate Wellness Membership Terms.

Members may cancel at any time through the website or by phone. Cancellation made before the next billing charge prevents the following month's charge.

Previously processed membership payments are nonrefundable, except where required by law or in cases of confirmed unauthorized, duplicate, or erroneous charges.

Enrollment requires affirmative authorization of recurring billing. Federal law requires clear disclosure of recurring-payment terms, express informed consent, and a simple mechanism for stopping recurring charges.

The Wellness Membership is not health insurance and does not replace health insurance.
 

6. Supplements and Medications

For health, safety, quality-control, and product-integrity reasons, supplements, prescription medications, injectable products, and other medication-related products are final sale once dispensed or purchased, except where otherwise required by law.

Products that have left our control cannot be returned for resale or redistribution.

If you believe you received an incorrect, damaged, defective, or improperly dispensed product, contact Let It Be Wellness promptly so the issue can be reviewed.
 

7. Refunds

Except where otherwise stated or required by law:

• completed services are nonrefundable;

• supplements and medications are final sale;

• previously processed Wellness Membership charges are nonrefundable; and

• unused discounts or promotional benefits have no cash redemption value.

Nothing in this policy limits rights relating to unauthorized, duplicate, incorrect, or unlawful charges.
 

8. Insurance

Unless specifically stated otherwise, services may be offered on a self-pay basis.

Let It Be Wellness does not guarantee that any service, product, membership fee, or treatment will be reimbursed by an insurance company, Medicare, Medicaid, HSA, FSA, or another payer.

Membership discounts or credits will not be applied in any manner that violates applicable federal or state healthcare billing requirements.
 

9. Privacy and Health Information

Personal information is handled according to our Privacy Policy.

Where Let It Be Wellness is subject to HIPAA, protected health information is handled according to applicable federal privacy laws and our Notice of Privacy Practices.

HIPAA-covered providers must maintain a Notice of Privacy Practices and prominently make it available through any website providing information about their services or benefits. 

Medical information should be submitted only through communication methods approved by the Clinic for patient information.
 

10. Email and Text Communications

By providing contact information, you may receive communications necessary for appointments, purchases, account administration, membership billing, and treatment-related matters.

Consent to receive promotional text messages or automated marketing communications will be obtained separately where required by law.

You may opt out of marketing communications using the instructions provided with those communications. Opting out of marketing does not necessarily stop essential transactional or healthcare communications.
 

11. Electronic Transactions

You agree that agreements, disclosures, authorizations, and records may be presented electronically where permitted by law.

Arizona recognizes the legal validity of electronic records, electronic contracts, and electronic signatures.
 

12. Website Use

You agree not to:

• use the website for unlawful or fraudulent activity;

• attempt unauthorized access to website systems or patient information;

• interfere with website operation or security;

• impersonate another individual;

• submit false medical, billing, or identifying information;

• reproduce or commercially exploit website materials without authorization; or

Nothing in these Terms waives or limits liability or patient rights that cannot lawfully be waived, including rights relating to professional negligence, medical malpractice, gross negligence, fraud, privacy violations, or statutory consumer protections.

Let It Be Wellness seeks to make its website and services accessible to individuals with disabilities.

The ADA applies to doctors' offices and other businesses open to the public, and the Department of Justice states that accessibility requirements extend to goods and services offered through websites.

These Terms are governed by the laws of the State of Arizona and applicable federal law.

Except where applicable law provides otherwise, disputes relating solely to these Website Terms will be handled in an appropriate court located in Maricopa County, Arizona.

Nothing in this section prevents a consumer from exercising any nonwaivable rights available under
applicable law.

21. Severability

If any provision of these Terms is determined to be invalid or unenforceable, the remaining provisions will continue in effect to the fullest extent permitted by law.
